If you have dental insurance, it’s important that you provide us with your plan information prior to your visit. As a courtesy, our front office team will use the information you provide to learn more about your plan’s eligibility requirements, coverage limits, copays, and more. This enables us to provide you with more accurate treatment estimates for any recommended dental procedures. To make your visit as comfortable and efficient as possible, you’ll find links on this page for your new patient forms and documents. Completing these forms in advance will enable you to provide us with details about yourself such as your contact information, insurance carrier, and medical history prior to your visit.

If you are unable to complete these forms in advance, please plan to arrive 20 minutes prior to your scheduled appointment and our front office team will be happy to assist you.

To ensure a smooth, stress-free appointment, we ask that you arrive on time for all appointments. If you are unable to make your scheduled appointment, we kindly request 24-48 hours' notice so that our team can prepare accordingly.

This visit is designed to get you acquainted with our office, Dr. Tim, Dr. Wajdi Alsabia, and our team. We’d like to talk about your long- and short-term dental goals, check for underlying dental problems, and complete a thorough dental screening and cleaning. If additional dental treatments are needed, we’ll explain it all in detail, provide you with pricing estimates, and prioritize your upcoming visits. If you have dental insurance, please bring your policy information and personal identification so that we can provide you with a treatment estimate.

If your dental needs are immediate, our team is here to help make you comfortable as soon as possible—especially if you are in pain. We may take an x-ray of the problem area and provide solutions for resolving the issue. Then, once you are no longer in pain, we can plan for a more thorough, comprehensive visit. If you have dental insurance, please bring your policy information and personal identification so that we can provide you with a treatment estimate.
